Required Physical Examination
Effective July 1, 2018 – New York State Education Law requires new entrants into the Wappingers Central School District and each student in grades Kindergarten, 1, 3, 5, 7, 9 and 11 to have a physical examination.
Please send a copy of your child’s physical examination, performed by your private physician on the attached MANDATED NYS School Health Examination Form, to the Health Office by October 1.

Parents are encouraged to have their sons and daughters examined by their family physicians. Results of the examinations must be noted on health forms furnished by the District, signed by a doctor and returned to the Health Office by October 1st.
If you have not provided the school with proof of a physical for your child, he/she will be scheduled for an examination in the near future by our school physician or one of his associates. The exams performed in school are the same as those done by your own physician. Any health conditions that seem to require more complete diagnosis or treatment will be referred to the parents.
