Effective July 1, 2018 – New York State Education Law requires new entrants to the Wappingers Central School District and each student in grades Kindergarten, 1, 3, 5, 7, 9 and 11 to have a physical examination on file.
Also mandated by New York State are health screenings. If your doctor completes any of the state mandated screenings on the physical exam form, then they will not be repeated at school.
Effective January 1, 2021 - all physical exams for public school students must be documented on the NYS Required Health Examination form. A "Visit Summary" is not a physical exam and will not be counted as one for the school year. A copy of this form is located on the bottom of the page for your convenience.
**Please Note: All physical exams must be completed on the NYS form. If a physical exam is not completed on the appropriate form, then the WCSD Medical director or one of his associates must repeat a physical at a later date. **
A Dental Health Certificate is recommended to be on file for 9th and 11th grade. However, it is not mandated to be on file.

-9th Grade: Physical Exam, Dental Health Certificate (optional), Scoliosis (boys only)
-11th Grade: Physical Exam, Dental Health Certificate (optional), Hearing, Near Vision, Distance Acuity
