In order to attend school in New York State, it is necessary that your child has the minimum required immunizations on file in the school Health Office.
The minimum requirements include:
DTaP or DTP (Diphtheria Toxoid containing vaccine) : 3 doses
IPV or OPV (Polio) : 3 doses
Measles : 2 doses ** (see below)
Mumps : 1 dose ** ( see below)
Rubella : 1 dose ** (see below)
**Usually given as "MMR" : 2 doses**
Hepatitis B : 3 doses
Varicella (Chicken Pox) :1 dose
Tdap (Tetanus, Diphtheria, Pertussis booster) : ONLY for 6th graders
These are New York State requirements. Your child will not be allowed to attend school without documentation of having received the required immunizations! The only exceptions allowed are a written medical exemption from your child's physician, or a religious exemption.
