In 2002 President Bush signed the No Child Left Behind Act (NCLB). This mandate has a provision that requires districts to provide, upon request, student names, addresses and telephone numbers to military recruiters.

To exercise your legal right to have this information kept private, you should use the form below to notify the school that you do NOT wish this information to be released. Please note that unless you use this "opt out procedure" the school district can not refuse the military recruiters' request and as a parent you and your child may be contacted by them at home.

Please complete this form ONLY IF YOU DO NOT WISH to have this information provided to military personnel. Please do so no later than September 30, 2024.
