As an incentive towards building our PTA membership, we are creating a Student Directory.  The directory will be given to PTA members only (PTA membership form ).  It will not be available for sale.  Each family, who chooses to fill out a form, will be in the directory – whether they are a member of PTA or not.
 

Please fill out one form per family.  If you would like to leave an area blank,
that is fine (i.e. – an unlisted phone number).  If your child has two addresses, list them individually.

 Please do not send in your PTA dues with the form.

Only the names and grades of the children attending Fishkill Plains will be in the book.  The directory will go to print at the end of October and handed out as soon as it comes in.
