Dental Health Certificate
New York State requests grades K, 2, 4, 7 and 10 students submit a Dental Health Certificate to the Health Office. [Optional]

FERPA
Family Educational Rights and Privacy Act of 1974


Health Examination Record
Print form only if a WCSD physical examination report is required (physicals are required for grades K, 2, 4, 7 and 10).  Usually the doctors use their own forms.

Enrollment Form-Residency Questionnaire*
 Print form only if a student is homeless.


Immunization Report
Print form only if a WCSD immunization report is required.  Usually the doctors use their own forms.

In-School Medication
Information explaining regulations for students who require medication during school.  Print form only if permission is required for school nurse to administer medication.

District Voter Registration Information

Print form and fill out the information regarding development history, health conditions, injuries, illnesses, surgeries and any additional information from parent for nurse.  

Home Language Questionnaire*
Print form and fill out the information which will determine how well your child understands, speaks, reads and writes English.

Kindergarten Home - Social Survey
For Kindergarten students only. Print form and fill out the information pertaining to child development.

Registration Data Sheet*
Print form and fill out the information regarding student’s name, address, parent contact information, siblings, previous school, verification of residency, country of birth, etc.  

Print form and fill out the release of student information.  Parent signature is required to request records from the previous school.  

Student Information Profile*
Print form and fill out the information pertaining to student’s academic, behavioral, and social/emotional strengths/needs.
