- Download either one of these application forms (both are in Adobe Acrobat format):
- Absentee Ballot Application, version 1 - to be filled out by hand.
- Absentee Ballot
Application, version 2 - to be filled out on your computer using
Adobe Acrobat Reader and then printed out.
- Mail the completed form to the District Clerk at the address shown below.
- Upon receipt of your application, the ballot will be sent to you with a return envelope.
- PLEASE NOTE: The ballot must be returned no later than 5 PM on the day of the vote.
- You may fill out an application at the District Office, at the address shown below, and vote at the same time.
- If you have questions, please contact the District Clerk.
